CPS is committed to addressing barriers to student health and wellness and providing education, programs, and activities that help improve the quality of life for students attending our schools. We also strive to decrease health disparities within the district through the coordination of school health programs and student health services.
Medical Compliance Notice
Physical examinations and/or required immunizations are due no later than October 15, 2024.
Who needs an updated physical and/or immunizations?
- Students in PE/Pre-K. (Lead screening required as well through age 6).
- Students in kindergarten. (Lead screening required as well through age 6).
- Students in 6th grade
- Students in 9th grade
- Students aged 5, 11, and 15 for un-graded programs
- Any student entering CPS for the first time
- Any student who is missing immunization from a previous school year
Families can get vaccines through Medicaid, federally qualified health centers, CPS school-based health centers, or a family’s health insurance plan. They can email oshw@cps.edu or call the Healthy CPS Hotline at (773) 553-KIDS (5437) if they need assistance.

Medical, Food and Other Public Benefits
CPS provides families with social services support, including Medicaid (health insurance) and SNAP (food stamps) enrollment. We can connect you to free and low-cost food, healthcare, and emergency resources to keep your family healthy and safe.
School-Based Health Centers
School-based health centers are primary health care sites located in schools or nearby schools. Students can access health services at no out-of-pocket cost. Most services require parent or caregiver consent. Call ahead before making an appointment.
Community Health Clinics
Federally Qualified Health Centers provide comprehensive, culturally competent, high-quality primary health care services as well as supportive services. Centers provide services regardless of a patient’s ability to pay and charge for services on a sliding fee scale.
